Dear Student,
When you arrive for a clinical round/practical experience at Hillel Yaffe Medical Center, administrative onboarding will be done by the Human Resources Department.
A. Before being onboarded, you will need to submit the following documents:

Confirmation from the Health Office that you have received the required vaccinations – Part C confirmation.

Complete the questionnaire for obtaining a police criminal background check and send along with a photocopy of your ID card at least two weeks before the round begins. Click here for the form >>

Confirmation from the police that the adult has no record of any sex crime (men only). Submit to the police with the attached reference and ID card – click here for the form >>

Confidentiality and information security undertaking – click here for the form >>

Signature indicating that the Student Manual has been read – click here for the manual >>

Current confirmation of studies, if necessary.

Confirmation of insurance from the academic institution, if necessary.
All of the above forms should be e-mailed together, as a single PDF file to the following address: StudentTraining@hymc.gov.il
Please provide the following information in your e-mail: full name, ID number, academic institution at which the student is enrolled, field of study, dates of the round and department.
B. Onboarding of a student:
After receiving confirmation from Human Resources, schedule a date for onboarding, according to the start date of the round, which will be done in the Human Resources Department, including the issue of a badge.
At the end of the onboarding process, the student will receive a Student Onboarding Confirmation form, which they will also use for the next rounds.
Without presenting everything that is required, you will not be onboarded.
